Auto-login tokens (deep links for email/SMS) are a TODO comment in the codebase. This blocks end-to-end patient onboarding flows and any email/SMS-driven workflows.

36 controllers bypass Manager layer
Audit (Mar 14) found 36 controllers injecting Repositories or Services directly. Violates mandatory Controller → Manager → Service → Repository pattern. Blocks architecture compliance sign-off.

Nightly regression pipeline not running
